Abstract
Purpose: To report an algorithmic approach for management of post-refractive surgery glare & asthenopia.
Methods: Of the 4000 operated eyes, 56 eyes of 28 patients had glare & asthenopia after refractive surgery. Patients with normal refraction, dry eye tests, topography, aberrometry underwent detailed orthoptic evaluation. Vision therapy(VT)exercises were initiated in cases with vergence/accommodative anomalies.
Results: 68% had glare & asthenopia & 32% presented with glare. Evaluation showed convergence and accommodative anomalies in 24 & 12 eyes respectively, divergence insufficiency & fusional anomalies in 4 eyes each, non-specific vergence/accommodative anomalies (NSVA) in 10 eyes. After 21 days of office VT & 2 months home VT, symptoms subsided significantly. 2 eyes showed partial improvement & needed recorrection.
Conclusion: NSVA contribute to post-refractive surgery glare & asthenopia. Orthoptic evaluation with VT can attenuate these symptoms and prevent unnecessary resurgery.
