Abstract
AIM: Acute corneal hydrops is a rare complication of corneal ectatic illnesses that involves sudden-onset corneal edema caused by a rupture in the Descemet membrane which can cause vision loss and pain. WE report a case of acute hydrops following unilateral keratoconus. The incidence of unilateral keratoconus was 4.5%. Methods: A 14yr boy presented with sudden decreased in visual acuity with whitish opacity. Results: On examination 3x4mm lucomatous opacity, stain positive with keratometric reading of right eye was- K1=77D, K2=93D and left eye was-K1=45D, K2=46D.On anterior segment-OCT central corneal thickness was 836µm. Conclusion: Initial management of acute corneal hydrops is often conservative, as many cases resolve spontaneously within 2 to 4 months. Topical measures frequently include an IOP lowering, hypertonic saline, cycloplegia, steroid, and an antibiotic.
