Abstract

14 year old boy with no relevant past history presented with one day history of blurring of vision in both eyes .Ocular examination revealed BCVA in the right eye 6/36 and left eye 6/12.Anterior segment was normal with no signs of uveitis. Fundoscopy of both eyes revealed hyperemic optic disc with pockets of sub retinal fluid in peripapillary and foveal region extending temporally with RPE alterations.Fundus autofluorescence,FFA and ICGA showed pinpoint leaks with multiple hypofluorescent areas.SDOCT Spectralis revealed pockets of sub retinal fluid with diffuse choroidal and sub foveal choroidal thickening bilaterally.Diagnosis of Vogt Koyanagi Haradas Disease was made based on imaging findings.Patient was started on high dose steroids and immunosuppressants. He responded well to treatment with no evidence of any systemic involvement.Multimodal imaging can confirm clinical diagnosis of VKHD.Prompt treatment with high dose steroids can salvage vision in this blinding disease.
