Abstract
Aim: Report aphakic refraction technique(ART) for calculating IOL power in refractive surprise (RS) after phacoemulsification(PE). Case: A patient with prior LASIK (OU) was referred with RS of +5.0D in the OS after PE. A IOL of +12.0D had been implanted. Re-calculation with multiple post-LASIK IOL power calculation formulae gave average IOL power of +12.90D that did not account for post-op refraction (+5.0dsph). Topography showed decentered ablation only in OS. Hence, IOL was explanted, refraction done on the first post-operative day. IOL power calculated using ART formula with spherical equivalent of +9.25D was +16.6D. A +18.0D IOL was implanted. After 1month, corrected visual acuity was 20/20, N6 with +0.25dsph-1.0dcylx70 (add+3.0d). For PE in OD with no decentered ablation zone, post-LASIK IOL power calculation formulae was used with no RS. Conclusion: Decentered LASIK ablation can give RS even if post-LASIK IOL power calculation formula are used. In such cases ART is beneficial.
