Abstract
24 years male presented with redness in right eye(OD) with dimness of vision for 10 days, with prodrome of fever, knee pain and mucocutaneous lesions. He had hand movement vision OD, panuveitis and dense vitreous exudates with no view of fundus. Multidepartmental consultation led to diagnosis of Behcet’s disease and systemic corticosteroids were started.
He had a febrile episode, blood culture showed presence of Sphinogomonas paucimobilis. USG abdomen revealed subclinical cystitis. Clinical diagnosis of Endogenous Endophthalmitis(EE) was made. Intravitreal antibiotics+steroids with systemic antibiotics, yielded favorable results with vision of 6/24 OD in a week. Vitreous tap was dry. Fundus examination, OCTA after reduction of the vitreous haze showed findings in favor of EE. The mucocutaneous lesions were fixed drug eruptions.
EE is an ocular emergency which is often masqueraded by other etiologies of panuveitis and prompt diagnosis and treatment is imperative for good visual prognosis
