Abstract
Axenfeld anomaly is a inherited autosomal dominant disorder characterized by anterior segment dysgenesis. Here, we present a case of 38 years male presented to OPD with progressive diminution of vision in left eye. Visual acuity was 6/9 in right eye and 6/36 in left eye. On slit lamp examination, there was posterior embryotoxon in each eye with iris atrophy, irregularly eccentric fixed pupil and ectropion uveae in left eye. Gonioscopy revealed anteriorly displaced Schwalbe’s line and strands of iris tissue extending into angle. On fundoscopy tessellation was present in each eye with vertical cup: disc ratio of 0.8-0.9 , thinning of neuroretinal rim and nasalization of vessels in left eye. Red free fundus photograph of left eye shows complete loss of retinal nerve layer. He was prescribed IOP lowering drops and planned for trabeculectomy under mitomycin C left eye under guarded visual prognosis. He was advised monthly follow up.
