Abstract
Blunt trauma to the eye is an underreported cause of loss of vision. We report a case of blunt trauma causing nasal cyclodialysis and dislocation of the lens in the nasal anterior chamber angle. A 40 year, one eyed male, presented with sudden, painful, diminution of vision in his previously seeing eye following a history of fall 1 day back. He presented with a visual acuity of PL+, PR accurate, a dislocated cataractous lens in nasal angle, distorted corneal contour, scleral thinning, hypotony and vitreous haemorrhage. The lens was removed through a temporal corneoscleral tunnel followed by pars plana vitrectomy and laser. He was kept aphakic. Secondary IOLs were not feasible due to thinned out sclera and deficient iris. Even though the outcome is far from perfect, we have achieved an ambulatory BCVA of 1/60 in a one eyed patient with surgical intervention and conservative management in the form of aphakic glasses despite the highly astigmatic cornea with a central corneal opacity.
