Abstract
Case report-59 year old woman presented with pain,redness,progressive axial proptosis in left eye and headache since 1.5 month.She was hyperstensive since 4 year.Best corrected visual acuity in left eye was 6/24(p) with axial proptosis,lid edema,chemosed conjuctiva,engorged episcleralvessels,shallow anterior chamber,fixed dilated pupil and painless restricted ocular movement.Left eye Intraocular pressure was 21.9 mmhg and fundus showed tortous blood vessels and splinter hemorrhages inferior to disc.Her MDCT orbit with contrast showed left Cavernous sinus thrombosis(CST)with indirect Carotid cavernous fistula and CEMRI Brain + orbit showed bilateral sinusitis,left CST with left orbital cellulitis.She was treated with prolonged duration of parenteral wide spectrum antibiotics,anticoagulants and local instillation of antibiotic and lubricating eye drop.Patient recovered well.
Conclusion-Timely diagnosis and treatment can reduce morbidity and mortality in complicated CST.
