Abstract
An 8 month old baby was brought by her parents with complaint of swelling just below right medial canthal region since birth. The baby was undergoing lacrimal sac massage for NLD obstruction. Swelling was gradually increasing in size. On examination, a bluish mass was noted below right medial canthus. It was non-tender, soft in consistency, smooth in outline. There was no matting of lashes, ROPLAS was negative, DDT was normal and there was no history of respiratory distress or painful episodes. Keeping in mind the differentials of Dacryocystocele, Dermoid cyst, Infantile Hemangioma, an MRI orbit was done which showed well defined lesion hyperintense on T2 and hypointense on T1 with flow voids suggestive of Capillary Hemangioma. Baby was sent to Paediatrician for systemic Propranolol and monitoring, to which she responded favourably. Thus, infantile Hemangioma should be kept as a differential diagnosis in cases of periocular subcutaneous swelling in the lacrimal sac region.
