Abstract
A 20 year old female presented with dimness of vision(VA 20/120) , 6 months post pterygium surgery in her right eye. Pterygium graft was in situ and anterior segment was within normal limits. Fundus examination in her right eye revealed white scar in macula with early epiretinal membrane(ERM) formation which was confirmed on Optical coherence tomography. On indirect ophthalmoscopy, a sealed scarred wound was found in superior retina. A provisional diagnosis of CNV with ERM was made secondary to iatrogenic double perforation of needle during local peribulbar anaesthesia prior to pterygium surgery. Fluoroscein angiography showed staining of macular scar with adjoining CNV leakage confirming our diagnosis. Barrage laser was done to the perforation site in the superior retina and traumatic secondary CNV was treated with 3 intravitreal anti VEGF(vascular endothelial growth factor) injections, following which visual acuity in her right eye improved to 20/30 with CNV regression.
