Abstract
Incidence of rhino-orbital mucormycosis is increasing in the current situation of SARS-CoV-19 in India and elsewhere. Neutropenia, diabetic ketoacidosis, organ transplantation, corticosteroid use and increased serum iron comprises various risk factors.. There are mainly two mechanisms via which mucormycosis spreads and grows – soft tissue invasion and angioinvasion .
Angioinvasion by hyphae results in occlusive vasculitis with infarction of orbital tissue .Infarction superimposed with aseptic necrosis is responsible for classic black eschar that may develop on palate ,turbinate ,nasal septum ,skin and eyelids .
Our Patient is non-diabetic with controlled blood sugar levels without any associated co-morbidities . He is a known case of RT-PCR positive SARS-CoV-19 . In our case ,necrosis initially involved lower lid margin and further extended to involve whole cornea .
It was surprising for us to see corneal eschar in a known case of rhino -orbital mucormycosis.
