Abstract
48 year old male presented to us with complaint of a sudden visual decline in his right eye(RE) and best corrected visual acuity(BCVA) of 6/18.On examination,there was a large horseshoe tear with bullous retinal detachment involving the macula.Patient next day underwent scleral buckling surgery with subretinal fluid(SRF) drainage and cryopexy.Postoperatively retina was well attached with BCVA of 6/18.Since BCVA was unchanged on 1 week follow up(F/U),Optical coherence tomography(OCT) was done which revealed shallow multiple pockets of SRF subfoveally and in superotemporal quadrant with increase in choroidal thickness.1 month F/U,fundus examination showed multiple hypopigmented lesions corresponding to SRF pockets on OCT.3 month F/U,OCT showed a slight decrease in SRF but accentuated fundus lesions in a giraffe/leopard spot pattern.On OCT angiography,dark spots suggestive of choroidal ischemia were seen.Although difficult to manage such pockets of SRF may affect the final visual outcome.
