Abstract
A 62 year old male presented with decreased vision and glassy reflex in left eye for 3 months. The patient was operated for penetrating keratoplasty with scleral-fixated IOL 9 months back. He suffered a total graft loss after forceful Valsalva maneuver during defecation 3 months ago when a transparent tissue fell into patient’s hand with escape of fluid from eye. Examination revealed visual acuity of hand motion and an accurate projection of rays, total absence of corneal graft, 1 broken continuous 10-0 nylon suture, 4 intermittent sutures, exposed SFIOL optic, 360° peripheral anterior synechiae with absence of iris tissue behind IOL, thick vascularized retrolenticular membrane continuous with the host margins in all quadrants, lower IOP (digitally) and grossly anechoic ultrasound. SFIOL served as scaffold for this membrane which preserved ocular integrity and protected intraocular structures by completely sealing them from external environment.
