Abstract
Aim: To compare between two techniques to achieve successful capsulorrhexis in intumescent white mature cataracts to prevent Argentinian flag sign in manual small incision cataract surgery.
Method: Prospective comparative randomized study. Two groups of 25 cases each. Group A, deliberately small continuous curvilinear capsulorrhexis (CCC) that was secondarily enlarged, or a two-stage CCC. Group B, A 24-gauge needle entered through a side port to decompress the anterior and posterior intralenticular compartments followed by a standard size one stage capsulorrhexis.
Result: A 92 % success rate was achieved in group A while in group B 84% success was achieved (p=0.663). In type 1 intumescent cataract 100% CCC achieved, while in type 2 and type 3, 33.3% and 22.2% respectively cases show capsular tear and discontinuity of CCC.
Conclusion: Both techniques are equally useful to achieve continuous curvilinear capsulorrhexis in cases of white mature cataract with high intralenticular pressure.
