Abstract
To evaluate the real-world outcomes after managing cases of significant SMH with more than 4 disc diameters (DD). A retrospective study of consecutive 75 cases treated by vitrectomy and divided into 3 groups – Group A(<4 weeks, within macula / inferiorly, n=45), subretinal tPA, anti-VEGF, air with SF6 gas; Group B(4-8 weeks, beyond macula, n=20), subretinal tPA & SMH drainage either by retinotomy/temporal 180-degree retinectomy with silicone oil (SO) tamponade & Group C(>8 weeks, beyond macula, n=10), SMH removal with RPE-Choroid graft transplantations with SO done. BCVA, Optos, OCT, USG done. Complications analyzed. Significant visual improvement seen in Group A(p<0.001 ), B (p = 0.016 ) & C (p<0.001). Post-operative complications recurrent SMH(6.67% vs 15% vs 10%), hyphema(6.67% vs 15% vs 10%), hypotony ( nil vs 5% vs 30%), macular hole (11.11%, A), retinal detachment ( 2.22%, A). Surgical intervention in significant SMH is visually rewarding, though few complications may arise.
