Abstract
A 29yr male presented with right red eye after blunt trauma. No history of red eye, pain.VA was 6/6 OU.Torchlight showed subconjunctival hemorrhage in right eye.Slit-lamp revealed a 1.5mm long iris cleft in ciliary zone, starting 0.5 mm from root of iris till collarette at 8 O’clock hour with splitting of stroma exposing whitish collagenous tissue.Adjacent iris texture preserved. No transillumination defect seen.Left eye was normal without similar cleft. Both eye pupil reflexes,IOP,angles & fundi were normal.Diagnosis of partial iris stromal tear made & patient followed up monthly.Blunt trauma causes tears in pupillary ruff or root of iris or both.Tears in ciliary zone are unknown.Iris atrophy features thinning, loss of crypts/texture.Trauma with iris tears usually affects other structures but none in our case. Conclusion:Exposed collagenous tissue can be misdiagnosed as iris atrophy.Examination under magnification of both eyes is vital.Isolated partial iris stromal tear is rare & unreported
