Abstract
Retinal detachment(RD) in children with a pre-existing Glaucoma Drainage Device (GDD) poses a unique management challenge especially if the GDD is in the area of the break. This video will showcase a unique surgical procedure using the episcleral plate of the GDD as a scleral buckle for fixing the RD. A 6-year-old one-eyed girl with Primary Congenital Glaucoma presented with an inferior rhegmatogenous retinal detachment (RRD) in her only seeing eye. She had undergone implantation of an Aurolab Aqueous Drainage Implant (AADI), a non-valved GDD, similar in design to the Baerveldt Glaucoma Implant, in the inferonasal quadrant two years back. Removal of the GDD to facilitate retinal re-reattachment surgery would be risky given advanced glaucoma and the inevitable subsequent IOP rise. We successfully managed her RRD with a 277 tire using the existing GDD plate’s segment as an explant. 2 months later, her vision remains at 6/24, and IOP was 10.0 mm Hg without anti-glaucoma medications.
