Abstract
Papilledema can cause blindness in as high as 40% with the visual function loss being the ONLY and SERIOUS complication. The typical management begins with medications but if the patient is unresponsive then will require optic nerve sheath fenestration (ONSF), to salvage the nerve and thereby the visual function. It is important for every ophthalmology to know about ONSF, which though, a neuro-ophthalmic condition needs to be performed ONLY by an ophthalmologist. In this video, we shall vividly describe various methods of diagnosing papilledema, the meticulous planning and provide an algorithm for choosing the appropriate case for ONSF. The surgery of ONSF will be demonstrated in a detail showing appropriate landmarks to access the nerve, necessary instrumentation, and important tips. This video will highlight different cases to show the benefits and simplicity of this technique enabling surgeons to effectively perform ONSF when the vision is threatened in cases of papilledema.
