Abstract
He had history of Vitrectomy done for left eye 3 months back. Surgery had to be abandoned . Visual Acuity was light perception. Examination of left eye revealed hyper mature cataract with total retinal detachment and medium reflectivity echoes in subretinal space on B Scan ultrasound. A 25G pars plana lensectomy and vitrectomy with silicon oil was planned. Lensectomy could be completed very quickly. Intraoperative examination revealed total RD with PVR changes and a large break superiorly with rolled edges. Membrane peeling was initiated at posterior pole. At this point we could note a mobile mass in subretinal space. Subretinal nucleus was detected and managed by manipulating it out through break. Inferior retinectomy and ILM peeling was completed to reattach retina. Video demonstrates the technique to manage this unique situation and should be a good learning.
