Abstract

Juvenile X-linked retinoschisis is usually a stable disease rarely presenting as retinal detachment (RD). A 1.5-year male presented with macula involving rhegmatogenous schisis RD in the right eye (RE) and a stable schisis without breaks in the left eye (LE) . RE was taken up for lens sparing parsplana vitrectomy and LE was kept under observation. The video demonstrated the challenges faced during the surgery. Posterior vitreous detachment was challenging and multiple inner retinal holes and 2 outer retinal holes were seen intraoperatively. On attempting to settle the retina under air an outer retinal fold with intra-retinal fibrotic band appeared along the inferior arcades. It was ironed out manually with a soft tip cannula as it could not be unfolded with PFCL . Nasal fold could be flattened however temporal one was stiffer. Sub-retinal fluid was drained and all the holes were lasered followed by silicone oil tamponade. Post op examination revealed an attached retina.
