Abstract
The quest of a dedicated MSICS surgeon is to develop skills to remove all forms of nuclei efficiently with minimal damage to endothelium & reducing the risk of PCR through smallest incision possible(2mm-4mm) thereby minimising astigmatism.There are many techniques of Phaco fragmentation in AC in MSICS.Dr Akahoshi,stunned the world by his in the bag prechopping technique.But for majority of us the learning curve is steep.We in this video describe a new Universal Sickle shaped,elegant and sleek Prechopper which is easy & efficient in producing nuclear disassembly of all cataracts in the Bag.A Chang’s Horizontal Chopper is used to hook the equator of nucleus & a new designed Prechopper is inserted through main incision & both instruments are brought together to produce a full thickness nuclear crack.The nucleus is rotated again & prechopped repeatedly to obtain ‘Pie Shaped’ nuclear fragments.The division produced by this prechopper almost imitates fLACS, taming even a hard cataract.
