Abstract

The most difficult step while operating a case of cataract with total total posterior synechiae with thick pupillary membrane is the release of the synechiae and adhesions between iris and anterior lens capsule. as the iris is totally plastered to lens capsule throught.

here presenting an innovative technique to seperate the total posterior synechiae, by injecting viscoelastic substance after doing a peripheral iridectomy – visco seperation or visco dissection. A back door entry!!!. rather than trying to release the synechiae through the pupil. which works out quiet well, as chances of damage to anterior capsule is minimal. manipulation is minimal so less stress on the zonules.

the surgical outcome was good , with good visual recovery , and a stable iol.
