Life does give us a second chance, and it’s our duty to give it the best shot each and every time. This is a case of a young male, who developed a subluxated cataractous lens post blunt trauma. Uneventful phacoemulsification with a CTR ring and in-the-bag IOL was performed, and the patient regained good vision post-surgery. Unfortunately, the IOL was noted to be subluxated one-week post-op, and the patient had to be reposted for an IOL removal with a vitrectomy with a secondary IOL. The four-point fixation technique of scleral fixated secondary IOL with prolene sutures was used. A month post-op, the vision improved significantly and is stable since then. Adequate counseling, a crisp plan of action, and due diligence are extremely crucial in such cases.
