Abstract
Glued IOL’s are a valuable tool in every cataract surgeon’s armamentarium. But what happens when Glued IOL falls short? In this case, a small incision cataract surgery was planned for a black, rock hard cataract with pseudoexfoliation and a 5 mm pupil through a superior scleral tunnel. Intra-op, zonular weakness was noted and the entire nucleus bag complex was delivered. A Glued IOL was promptly performed during the course of which the surgeon realised that the white to white corneal diameter was large(12.5 mm) and there was less haptic left to tuck. This video is an attempt to demonstrate the technique of Vitrector-assisted peripheral iridectomy with exteriorisation of leading haptic through anterior sclerotomy at 0.5 mm instead of the usual 1.5 mm from the limbus.
