Abstract
A 62-year-old female, diabetic for 25 years and hypertensive for 10 years presented with decreased vision in both eyes, Right eye was affected more than left eye. Best corrected Snellen visual acuity in right eye was finger counting 3 meters and left eye 6/9. Patient had Proliferative diabetic retinopathy in both eyes with taut posterior hyloid and large macular hole in right eye. The same was confirmed on OCT. Left eye underwent PRP laser. Right eye underwent pars plana vitrectomy with C3F8 gas injection after initial intravitreal antiVEGF injection. At last follow up the vision had improved to 6/36 with attached retina and successful closure of macular hole.
Conclusions: Taut posterior hyloid in PDR with macular hole is a challenging situation. Meticulous and careful dissection can achieve good visual outcome in such patients.
