Abstract
A 52-year-old lady presented with redness, severe ocular discomfort, blurring and inability to open the eyes.Her best corrected vision was 6/60 (OU). On fluorescein staining, total epithelial defect was noted bilaterally. Despite topical lubricants and antibiotics, the epithelial defect persisted.BCL was then applied which improved her vision to 6/12 with reduction in epithelial defects.
She was post allogenic bone marrow transplantation (BMT) for AML and was on topical Cyclosporine and systemic steroids as prophylaxis for ocular and systemic Graft versus host disease.Chronic GVHD occurs in 80% of allogenic Hematopoeitc stem cell transplant recipients. Eye is involved in 80% of chronic GVHD, commonly involving the ocular surface, lacrimal glands, Meibomian glands and lacrimal drainage system, leading to persistent epithelial defects significantly affecting the quality of life.If left undetected and untreated, they can lead to corneal ulcer and perforation.
