Abstract
A 54 yr old Male patient, attended at our ophthalmology department referred by his endocrinologist for fundoscopy for diabetic retinopathy screening. On Ocular examination, BCVA in his right eye is (-1.25 Ds/-0.75 Dcyl x 90*) 6/6 and (-6.00 Ds/ -1.75 Dcyl x 45*) 6/24 in his left eye. At fundoscopy, Right eye was normal & left eye showed white striated opacities emerging from the optic disc. The axial length of his eyes was 22.39mm in right eye and 26.48mm in left eye. The OCT showed parafoveal hyper-reflective retinal nerve fiber layer in left eye and right eye was normal. The presence of myelinated Retinal nerve fibers is not a rare finding during routine ophthalmological examinations, and it is usually a benign and isolated finding. However, in some rare cases, it can be associated with other ophthalmological conditions like unilateral myelinated retinal nerve fibres (MRNF), high axial myopia, amblyopia and strabismus, which constitutes the Straatsma Syndrome.
