Abstract
We report 2 cases of fungal sinusitis associated with retrobulbar neuritis with no history of COVID 19.
Both cases presented with sudden loss of vision right eye associated with pain. Vision was light perception and pupil showed grade 4 RAPD. Both cases had raised blood sugars. CT chest of case 1 revealed features of sequelae of atypical viral or COVID pneumonia. Keeping retrobulbar optic neuritis as diagnosis, MRI was advised which was suggestive of acute invasive sinusitis in both cases with features of optic neuritis.
Invasive fungal sinusitis usually presents as orbital apex syndrome and multiple cranial nerve palsies. Since both of our cases presented as isolated retrobulbar neuritis, one should be cautious while treating it with steroids, considering the probable fungal cause. Mucormycosis cases have been on peak in this pandemic, and hence possibility of it causing retrobulbar neuritis cannot be denied. Thus, we must do the suggested workup to rule out mucor before treatment.
