Abstract
Rhino-Orbital-Cerebral Mucormycosis (ROCM) is an opportunistic fungal infection occurring in immunocompromised hosts and patients with uncontrolled blood sugar levels and has high morbidity and mortality despite treatment. Patient presents most commonly with sinusitis, facial pain and tenderness, periorbital pain and edema, cellulitis, external ophthalmoplegia, proptosis, ptosis and diminution of vision. Advanced cases present with complete loss of vision. Diagnosis depends on CT/MRI brain and orbit which shows involvement of paranasal sinuses and cavernous sinus. Histopathology and culture of biopsy specimen provides true diagnosis of the entity.Treatment includes multidisciplinary approach. There are conditions which clinically may present like ROCM but on investigation turn out to have a different diagnosis.
We report such a case of a 57 year old male presenting with clinical features suggestive of mucormycosis but later diagnosed with indirect carotico-cavernous fistula.
