Abstract: Orbital ischemic compartment syndrome is an uncommon ophthalmic surgical emergency. Orbit, being a closed cavity, rise of tension beyond the capacity to compensate results ischemic damage to the optic nerve and thus functional visual impairment. Post-operative vision loss along with ophthalmoplegia is a known complication of prolonged spinal surgery. We report a similar case of a patient who was referred to us following a spinal surgery done elsewhere. Both anesthetists and the surgeons play a role in prevention, prompt diagnosis and early treatment of this condition. Even though the prognosis is poor, early intervention may prevent irreversible visual loss.
