Abstract
A case report of ocular ischemic syndrome(OIS) associated with macular ischemia
74year male patient presented with right eye sudden diminution of vision past 4 days.He was hypertensive and cardiac patient not on treatment.Right eye vision was 2/60,IOP32mmhg. Anterior segment examination revealed corneal epithelial edema, neovascularization of iris, RAPD and pseudophakia.Gonioscopy showed blood in Schlemms canal.Fundus evaluation showed normal disc,dilated veins with multiple dot blot haemorrhages in all quadrants and cottonwool spots around macula.Left eye was within normal limits.Fundus fluorescein angiography(FFA) revealed delayed patchy choroidal filling,delayed arterial filling and enlarged FAZ with vascular pruning suggestive of OIS with macular ischemia.Patient was treated with PRP,Anti VEGF and topical glaucoma medications.OIS is a rare condition and is misdiagnosed as CRVO.Macular ischemia can be one of the features of OIS.Early diagnosis and referral of OIS can be life saving
