Abstract
Purpose: To compare the results of full tendon medial transposition of lateral rectus with that of split tendon medial transposition of lateral rectus for complete III Nerve Palsy. Method: Medical records of patients who underwent transposition of lateral rectus to medial rectus for complete III Nerve Palsy were reviewed. Results: There were 7 patients; 4 had undergone full tendon transposition(passed beneath SR & SO) and 3 had split tendon transposition(upper half passed beneath SR & SO, inferior half passed beneath IR & IO). The pre-operative deviations were more than 50 PD (range 45 – 90)PD in all patients. The success rate (horizontal alignment within 20PD of orthotropia) was achieved in all patients. No patients in split tendon group has vertical deviation but all patients in full tendon group had more than 10 PD hypertropia requiring further surgery. Conclusion: Split tendon medial transposition of lateral rectus achieved better alignment than full tendon medial transposition.
