Abstract

Management of Posterior Capsular Cataract is very challenging. Our study includes 18 eyes with PPC who underwent phacoemulsification. After CCC, trenching was done followed by “Inside-Out Hydrodelineation”. Hydrodissection is contraindicated, Hydrodileneation is mandatory to separate the nucleus and epinucleus. Vigorous decompression of the capsular bag is contraindicated. Of the 18 eyes, 17 eyes were done successfully, 1 had a small PCR which was managed well. Incidence of PCR in our study was 5.5%, other authors reported incidence between 7.1% to 36%. This type of cataract needs three steps-first, using a modified surgical “Inside – Out Hydrodelineation” second, paying great deal of attention to the floppy posterior capsule; third, avoiding direct dissection of the polar opacity from the posterior capsule during surgery. Our modification of delineating followed by viscodissection is extremely safe and reduces the risk of PCR making Phacoemulsification in a PPC a safe procedure.
