Abstract
64 yr old lady ,Post cataract sx day 1,she presented with redness,discharge & decrease in VA in her left eye.BCVA was 6/60.AS showed Limbus to limbus corneal edema, AC inflammation & hypopyon.IOP were 40mmhg.B scan -no vitreous echoes.She was diagnosed as TASS & started on intense topical steroids & antiglaucoma drugs.3 hrs later hypopyon increased (2mm) & vn dropped PL + & PR inaccurate.B scan -dot like echoes of moderate intensity in the vitreous cavity.Pars plana Vitrectomy +Vitreous biopsy+I/V Antibiotics was done. Intraop the disc was pale with occlusive arterial vasculopathy all over with pale appearing macula.Culture report -Klebsiella pneumoniae. Post op- vn ? PL &fibrinous membrane in the AC with hypopyon 3mm. B scan-Inferior vitreous cavity again filled with inflammatory material.Emergency IOL explantation+Revit+ Vitreouslavage+I/V Moxi+Silicone oil + temporary K pro +TPK.Patient symptomatically better.AS showed graft edema with membrane in the pupillary areas no hypopyon.
