Abstract
Purpose: To compare residual stromal thickness in eyes undergoing small incision refractive lenticule extraction using diameter of 6.5 vs 5 mm. Methods: Retrospective series from 2016 to 2021 with 6 months follow up. Preop BCDVA, ref error, contrast; CCT, keratometry, aberrations & scotopic pupil were recorded by Sheimpflug tomography. SMILE of 6.5mm performed till 2018 (n=372) thereafter with 5mm (n=318). The RST, postop ref, aberrations, glare & halos compared in 2 groups at 1 & 6 months. Results: Mean age 26.8 + 5.8 yrs with mean preop sph equivalent of -4.48D + 2.16D (-0.75 to -12.25D) & mean scotopic pupil of 3.7 + 0.75mm. The 5mm group had 30.6 microns (95% CI= 28 to 33 µ, p<0.001) greater RST compared to the 6.5mm. There were no differences in vision, contrast, aberrations (Δ Wavefront error of 0.19 + 0.2 vs. 0.25 + 0.2, p=0.19) & glare between the 2 groups. Conclusions: First study of SMILE with 5mm optic zone leads to greater RST without significant higher order aberrations.
