Abstract
a 12 year old male child came to the hospital with chief complaints of outward deviation of right eye since 4 years .His unaided vision was 6/6.on examination HCRT showing 25 degrees exo with alternate fixation. His PBCT values in primary position,up gaze and down gaze are 50,80,45 base in prism diopters respectively.On dextro and levoversion there is upshoot in adducted eye because of inferior oblique over action( grade 3 in right eye and grade 2 in left eye) .on fundus examination there is excyclotorsion of macula produced by the inferior oblique over action.we did bilateral 11 mm lateral rectus recession and asymmetric inferior oblique weakening procedures( in right eye -PARK’s recession recession and in left eye – FINK’S recession)On post operatively,his HCRT in both eyes were central and no torsion detected on fundus examination.With surgery, child’s v pattern collapsed. Cosmetic alignment achieved. BSV not attained as child presented late to the hospital.
