Abstract
Central retinal artery occlusion is analogous to acute stroke of eye and is an ophthalmic emergency. 23 year old male, a case of TOF on conservative management presented with complaints of defective vision Right eye for 3 days. He had BCVA of 6/9 in RE and 6/6 in LE. His anterior segment examination revealed RAPD in RE. Peripheral field defect in RE, IOP and colour vision were normal. RE fundus showed minimal disc margins blurring, venous tortuosity, mild arteriolar attenuation, cloudy swelling of posterior pole sparing the papillomacular area suggestive of CRAO with cilioretinal artery sparing. His hemoglobin is 16.5 g with normal platelet and WBC count. Peripheral smear showed Polycythemia which was Secondary to underlying hypoxemia in cyanotic heart disease. homocysteine level was elevated , other blood parameters were normal. Carotid venous Doppler showed no occlusion. He was started on antiplatelets, anticoagulants, folate and vitamin B12 supplements after hematology opinion.
