Abstract
Aim: To report a case of autoimmune necrotizing sclerokeratitis secondary to lepra reaction.
Material: A 35 year old male presented with dark patches over right eye. He developed lepra reaction to multidrug therapy for hansen’s disease along with redness and pain that responded to oral steroids. Ocular examination revealed best corrected visual acuity of 6/12 in both eyes along with large areas of scleral necrosis. Patient was on irregular follow up and poorly compliant to medication causing extension of scleral necrosis and development of complicated cataract. Visual recovery after clear corneal extracapsular cataract extraction with intraocular lens (IOL) was 6/12 both eyes but the disease extended in the right eye causing corneal sloughing with IOL extrusion.
Conclusion: Lepra reaction can cause autoimmune necrotizing sclerokeratitis with grave sequelae if not managed properly.
