Abstract
64 year old female presented with gradual progressive diminution of vision in both eyes since 4 weeks. She underwent both eyes uneventful cataract surgery 3 years back. Patient is a known case of systemic hypertension and hypothyroidism on treatment. She mentions receiving Ozurdex Implant (Dexamathasone 0.7 mg) in both eyes 2 years back and intravitreal Triamcinolone Acetonide (4µg) a year ago in either of the eyes. Her BCVA was 6/36 in both eyes. Anterior segment examination was within normal limits,with good pseudophakia. Fundus examination showed presence of Cystoid macular edema and was confirmed by SS-OCT. Due to its chronicity,we decided to administer Intravitreal Brolucizumab(6mg) in the right eye. At 1 month CME in the right eye reduced by 59 µm (403-344) with no visual improvement. However, to our surprise left eye showed improvement in vision to 6/18 with reduction in CME by 111 µm (399-288). At 6 months, the vision was 6/18 in both eyes with persistent macular edema.
