Abstract
Introduction. Mucoceles of the paranasal sinuses are benign expansive cystic lesions that occur rarely in the sphenoid sinus and contain mucous material enclosed within cylindrical pseudostratified epithelium. Objective. To report a case of sphenoid sinus mucocele that presented with visual loss. It was managed surgically through an endonasal endoscopic approach. Case Report. A 49 year old female patient with decreased perception of light for a month which worsened over 2 days with associated pain. Afferent pupillary defect along with disc pallor was noted on examination. MRI scans confirmed a uniform hyperechoic expansive mass of sphenoid sinus, suggestive of a mucocele. Patient was started on steroids and via endonasal approach sphenoidotomy with marsupialization was done. Conclusion. Mucoceles of the sphenoid sinus are a very rare with variable clinical and radiological presentation. Surgical treatment is absolutely indicated to avoid permanent visual damage.
