Abstract

Linezolid is one of the drugs used in the treatment of MDR-TB which is known to cause optic neuropathy. A 40-year-old male patient presented with gradual, painless diminution of vision in both eyes for 25 days. He was receiving treatment for MDR-TB since 7 months. Visual acuity was counting fingers 2m in both eyes. Colour vision was abnormal in both eyes. Anterior segment was normal with brisk pupils. Fundus examination showed temporal disc pallor in both eyes. Visual field analysis by HFA showed bilateral peripheral field constriction. OCT showed bilateral RNFL thinning.VEP showed bilateral prolonged P100 latency and reduced amplitude. Linezolid-induced toxic optic neuropathy was suspected and Linezolid was discontinued. After 4 weeks colour vision was restored and vision improved to 6/24.Physicians and ophthalmologists must be aware of the importance of monitoring visual function in patients on linezolid therapy, which can help in early detection of toxicity.
