Abstract

PURPOSE- to report a case of 43Y/M presenting with decreased vision with history of ATT

METHODS- 43Y/M presented with painless, gradually progressive decrease in vision in BE for 3 months. He had been taking ATT till 1 month back. BCVA was 6/60 BE. Color vision test showed blue-yellow defect. Anterior segment normal, EOM full, IOP normal. On fundus exam, OD showed mild temporal pallor with CDR 0.5:1. Humphrey VF test, VEP and MRI scan were done.

Patient was started on vitamin B complex

RESULT- Humphrey VF test showed bitemporal hemianopia. VEP revealed increased P100 latency. MRI scan was normal. History and tests suggested diagnosis of ethambutol related optic neuropathy.

After 2 months follow up BCVA improved to 6/36 in BE with improvement in color vision

CONCLUSION- Patients receiving ATT needs to be screened for adverse effects of anti tubercular drugs. Early diagnosis and treatment provides better prognosis and prevents permanent vision loss.
