Abstract
A 32 year old male presented with right eye metamorphopsia of 2 months duration. His visual acuity in the right eye was 6/9, with presence of a perifoveal temporal cuff of subretinal fluid with multiple pigment epithelial detachments. The patient responded spontaneously without intervention. 4 months later, the patient had a new area of subretinal fluid nasal to fovea which responded to eplerenone, but recurred in a third location 2 months later. The fluorescein angiogram and ICG showed RPE window defects. The patient was lost to follow up for 2 years, following which he presented with a subfoveal detachment. At this visit, the patient complained of a left flank pain which showed a paraaortic mass lesion, biopsy of which revealed a seminoma. The patient gave history of undescended testis for which he underwent orchidectomy. A diagnosis of Acute Exudative Polymorphous Paraneoplastic Vitelliform Maculopathy was made, and the patient was advised chemotherapy for the metastasis.
