Abstract
Corneal hydrops in cross-linked cornea is a rare special scenario and we describe the clinical features and management of same. A 30-year-old male with keratoconus, underwent accelerated collagen crosslinking (ACXL) in the right eye. Patient’s ectatic status was stable for 7 years post-operatively. However, after the 8th year of ACXL, topography started showing signs of progression in the crosslinked eye probably because of weaning effect of crosslinking and collagen turnover after 7 years, hence re-crosslinking surgery was advised. Patient was lost to follow up for one and half years and presented in emergency with corneal hydrops in the right eye. Compression sutures with intracameral gas injection was done immediately with good surgical outcome and visually rehabilitated with scleral CL to 6/9 best corrected vision. This case highlights the first ever occurrence of corneal hydrops after ACXL for keratoconus and its successful management.
