Abstract
Sympathetic Ophthalmia is a rare bilateral diffuse granulomatous intraocluar inflammation occurring after an inciting penetrating injury or surgery to one eye.A 32 year old male presented with redness, pain and sudden diminution of vision in the RE, with a history of scleral tear repair for LE 2 months prior. The patients’ VA in the RE was 6/6, with normal retina at the time of repair. On clinical examination at time of presentation, VA (RE) was Finger counting 3 meters, with conjunctival injection, RPE alteration, and DalenFuch’s Nodules with large SRF at macula. The LE was pre-phthisical.Treatment was started; IV Methylprednisolone 1gm/day for 3 days, and tapering oral steroids over 3 months.Within 3 days, there was a notable reduction in SRF.Patient recovered with unaided VA of 6/9 (RE).This case highlights the need for a keen eye in diagnosing SO, prompting early intervention to salvage the vision of a single vision providing eye of a patient to prevent complete blindness of both eyes
