Abstract
Aim: To develop clinical scoring system for diagnosis of Pythium keratitis(PyK). Method: Clinical signs of PyK(n=27) were compared with fungal keratitis(n=69). Receiver operating characteristic curve was plotted for different signs to calculate area under the curve (AUC). Results: AUC and 95% CI: full-thickness stromal infiltrate (0.61 (0.48 – 0.73), dry surface (0.66 (0.54 – 0.78), tentacles (0.77 (0.65 – 0.90), intrastromal dot lesions (0.62 (0.49 – 0.76), ring infiltrate (0.57 (0.44 – 0.70), reduced infiltrate in the center (0.68 (0.54 – 0.81), reticular pattern (0.71 (0.58 – 0.84) and peripheral furrow (0.64 (0.47 – 0.74). A score of +1 was allotted to each sign. An aggregate score of 2 or more had 70.4% sensitivity and 5.8% false positive rate while a score of 3 or more had 48% sensitivity and 2.9% false positive rate. Conclusion: The clinical scoring system can aid ophthalmologists differentiate PyK from fungal keratitis, guide microbiology workup, and avoid delay in treatment
