Abstract
Orbital cysticercosis affects the extraocular muscles, involvement of optic nerve is rare and presents as optic neuritis. Here we present a case of 38-year-old lady who came with complaints of left eye sudden painless drop in vision since 10 days. Her presenting BCVA was 20/20 and counting fingers in right and left eye respectively. Examination revealed RAPD in left eye and HVF showed temporal field loss. MRI showed a well-defined hypo-intense lesion with hyper-intense margins in the substance of left part of chiasma. The differentials of optic chiasma cysticercosis, cavernoma, glioma were thought of and a CT was obtained which showed well defined Isodense lesion with surrounding hyperdense wall and central hyperdense dot likes lesion suggestive of cysticercosis of optic chiasma. Neurologist review was sought and started on oral steroids. This case highlights the rare occurrence of cysticercosis of optic chiasma which should be highly suspected in cystic lesion of optic pathway.
