Abstract
56 years old female on AKT for glandular TB complained of gradual, progressive, painless diminution of vision in the left eye since 1 month- BCVA of OD 6/12, OS 6/24, myalgia with occasional evening fever. The anterior segment evaluation only showed iris pigments on the lens and dilated fundus showed multiple, active choroiditis lesions on the posterior pole of both eyes with a small area of subretinal fluid at the macula of the left eye with no vitritis. she tested positive for IgG of herpes simplex I, toxoplasmosis, cytomegalovirus, and rubella; also IgM positive for herpes simplex II and strongly Mantoux positive, negative for HIV. FFA and B-scan were done and she was treated with IV Methylprednisolone for 5 days, with slow tapering of oral prednisolone over 6 months, 9 months AKT, oral antivirals, and a multitude of other antimicrobials. a gradual recovery of vision to 6/6 in both eyes was finally achieved over 6 months, with only remnant healed scars and no SRF, documented on FFA
