Abstract

We demonstrate intraoperative approach to phaco- triple procedure with minimal open –sky time. Our patient aged 17 years presented with defective vision & an opaque failed graft with vision of 1/60 in one eye. Anterior chamber was deep with a superotemporal large peripheral iridectomy& an associated PSCC.IOP was 20mm Hg. A phacoemusification procedure was performed assisted by 23G endoilluminator, which aided to visualise the anterior chamber details, despite dense corneal opacity. A foldable IOL placed in the bag, followed by full thickness keratoplasty.

Endo- illuminator is a simple, yet effective aid in the management of cataract associated with poor visualisation. This technique significantly reduces intraoperative open sky duration & thus the anticipated complications. Our current case was a planned Re-graft with marginally high IOP under medical management. Ensuring least open – sky time is crucial in such cases.
