Abstract
A 35 year old man with history of penetrating ocular trauma presented with pain and redness of 2 days’ duration. He had dense vitreous hemorrhage and a metallic intraocular foreign body (IOFB) in the inferior quadrant was visualised by imaging. Lens extraction with 23G pars plana vitrectomy was done after which localised frosted branch angiitis (FBA) like vasculitis was noted adjacent to the IOFB. Site of IOFB entry and impaction were lasered. Over a PFCL cushion, IOFB was removed by handshake technique via the anterior chamber after a failed attempt initially. Corneal wound was sutured and PPV ports closed after performing fluid-air exchange. The removal of IOFB with continued use of systemic and topical steroids post-surgery resulted in complete resolution of vasculitis within a week and improvement in vision. Frosted branch angiitis is an immunological response to numerous antigens. We suspect an association of retained metallic IOFB with the development of frosted branch angiitis.
